Ohio’s Hospice LifeCare is a not-for-profit, community-based hospice that serves Wayne, Holmes, Ashland and Medina counties. Hospice care can be provided in the patient’s home, an assisted living setting, a hospital or a nursing home. When appropriate, patients may be admitted to our inpatient unit located in Wooster, Ohio.
